ElderAudience Agreement with Entertainer/Service Provider

At your request, ElderAudience will display on its website your name as the Entertainer/Service Provider, contact information, a short description of the entertainment/service you provide, as you set forth below, and, if you provide them, a digital photograph and a link to an externally hosted video, e.g., YouTube or Vimeo. ElderAudience reserves the right to edit the information it hosts on its website, in its sole discretion, to meet its needs for space and to maintain the integrity of the site. ElderAudience takes no responsibility for the functionality, accuracy, or compatibility of externally hosted videos and it is Entertainer/Service Provider’s sole responsibility to maintain a relationship in good standing with the video-hosting service. It is Entertainer/ Service Provider’s responsibility to immediately notify ElderAudience if there is any incorrect information at Entertainer/Service Provider’s listing. ElderAudience will, as soon as practicable, modify the information on Entertainer/Service Provider’s listing, at Entertainer/Service Provider’s written request. ElderAudience has no liability for any incorrect information on the Entertainer/Service Provider’s listing.

Upon receipt of payment as set forth below (or approved exemption as volunteer/donation only-service provider), and listing information and its confirmation and approval, the Entertainer/Service Provider’s listing will be posted to the ElderAudience website within 7 working days. If you, as the Entertainer/Service Provider, decide you would like your listing removed from the Website, ElderAudience will do so as soon as it is reasonably able, but in any event within 10 business days of receipt in writing of your request for removal.

ElderAudience reserves the right to decide, in its sole discretion, whether Entertainer/Service Provider will be, or is allowed to continue to be, listed on its website. If ElderAudience decides to remove an Entertainer/Service Provider’s listing from its website, or if ElderAudience needs to provide any other notice to the Entertainer/Service Provider, it will give notice of that fact to the Entertainer/Service Provider within a reasonable time, at the Entertainer/Service Provider’s address or at the /Service Provider’s email address, if an email address is listed, below.

Any dealings or resulting agreements between a senior facility and Entertainer/Service Provider for the Entertainer/Service Provider’s services are theirs alone; ElderAudience plays no part in them. ElderAudience makes no representations whatsoever regarding the quality or business bona fides of either the senior facilities or the entertainers/service provider who might use its services.

Any written notice that needs to be provided to ElderAudience under this Agreement will be deemed to be from the Entertainer/Service Provider if it is in writing and signed by the person signing this Agreement, or if it is received via email from the Entertainer/Service Provider’s email address set forth below. Payment: To be listed as an Entertainer/Service Provider, payment of the Annual Fee must be received by ElderAudience unless Entertainer/Service Provider qualifies for a free listing as a volunteer or donation-only provider. Payment of the Annual Fee (or approved exemption) entitles the Entertainer/Service Provider to a listing on the ElderAudience website for one year. Pro rata refund of the Annual Fee will be made if ElderAudience decides to discontinue Entertainer/Service Provider’s listing prior to the expiration of one full year.

Volunteer entertainers/service providers or those who only accept donations for their services who are approved for listing on ElderAudience will be listed for no charge.

Method of Payment: Payment may be made 1) Digitally via debit/credit/bank transfer using Quickbook Payments. (Currently available on the invoice that will be emailed to you; 2) by check to ElderAudience, at address shown above; or 3) No payment is being made because I am a volunteer Entertainer/Service Provider who accepts no payment or who only accepts donations for my services.
